Esse Health is currently seeking a PRN Registered Nurse to work within our Hospital at Home department.The Registered Nurse provides expert professional nursing care; assesses, plans, implements and evaluates the care rendered to patients with urgent, emergent or potential health problems.

Qualifications:•Education: Formal training which will be indicated by a graduation certificate from an accredited RN school of nursing.•Licensure/Certification: Current State Licensure; CPR certificate.•Valid driver's license and proof of auto insurance•Years of Experience: 1-2 years preferrable in home care, home infusion, ED or ICU

Knowledge, Skills & Abilities:- Experience in Home Care or Home Infusion Nursing- Works directly with patients to provide assessment, plan of care, and interventions as part of a multidisciplinary care team- Coordinates referrals and resources to Ancillary Service Providers to quickly assess and treat complex conditions- Ensures all orders are executed to provide timely care and results daily- Ability to initiate Peripheral IV's, manage PICC lines, access Ports and other central venous lines- Ability to provide wound care within nursing scope of practice- Communicates changes in conditions to providers to assist with interventions and treatment- Ability to utilize personal automobile for visits to patient home- Ability to participate in on call, day and evening shifts and on call rotation- Dedication to patient satisfaction- Ability to communicate effectively- Ability to work compatibly with physicians and other staff members

1. Triage patient questions/ problems; consult with physician and return calls.

2. Assist physician and ensure maximum efficiency; set up for procedures and examinations; obtain lab test and other reports for physician review.

3. Triage patients to ensure productive departmental flow; revise daily schedule;

4. Record health history of patients; use assessment skills to note abnormal conditions for physician; prepare patients for exams and procedures.

5. Administer medications as appropriate, document in patient's chart.

6. Collect, label and document lab specimens.

7. Notify patients of test results, schedule follow-up appointments as appropriate.
