Position Summary :
The Harm Reduction Clinic Nurse Practitioner (HRC NP) provides medical care and treatment to patients of Brockton Neighborhood Health Center's Harm Reduction Clinic, an office-based addiction treatment (OBAT) program. The HRC NP will diagnose and manage substance use disorders and comorbid mental health conditions in a low-barrier, harm reduction-focused outpatient treatment program. The HRC NP will serve as an integral member of a multidisciplinary team that provides substance use treatment in a large, federally qualified health center in downtown Brockton. They are champions of collaborative, patient centered care that is inclusive of harm reduction strategies and medications for addiction treatment (MAT).
Principle Duties and Responsibilities:Provides direct patient care for substance use and co-occurring mental health disorders, both in-person and via telemedicine, that is inclusive of a harm reduction frameworkWorks collaboratively as a part of a multidisciplinary team including nurse care managers, behavioral health clinicians, recovery coaches, and other support staffProvides counseling, intervention and advocacy to all patients taking into account their unique cultural backgrounds. Education around issues such as high-risk behavior, mental health, substance use, and domestic violence is also a major component of job dutiesEvaluates patients by collecting a comprehensive assessment of each client, which includes mental status, current functioning, medication history, treatment history,Initiates medication for addiction treatment (MAT) including medications such as buprenorphine, naltrexone, acamprosate, as well as other drug therapies for patients served in the Harm Reduction ClinicMaintains a practice that is culturally and linguistically sensitive, using interpreters and other assistance as appropriateRefers patients, as appropriate, to specialists and other practitioners and facilitiesProvides intervention and referral for patients experiencing suicidal/homicidal ideationCompletes all clinical documentation in a thorough and timely mannerOrders and interprets screening and diagnostic testsCommunicates effectively and respectfully with all patients and coworkers in a timely and appropriate mannerMaintains productivity as outlined in contracted agreementMaintains patient's right to privacy and confidentiality in accordance with HIPAA and 42CFR Part 2Provides technical assistance and training for other members of the care team regarding medications, substance use disorder, and mental illnessMaintains clinical competence through ongoing medical education and trainingAttends all required and pertinent meetingsParticipates as an integral part of an interdisciplinary addiction care team including high-risk case review, team meetings, and protocol and procedure developmentQualifications:

Current Licensed Massachusetts Nurse Practitioner in good standingBoard certified Nurse PractitionerCurrent DEA Registration with DATA Waiver to prescribe buprenorphine, preferably up to 100 patientsCertified Addictions Registered Nurse (CARN or CARN-AP) strongly preferredBLS CertificationBilingual preferred
WORKING CONDITIONS
:
Works in a busy community health center in a downtown business district. Long hours are sometimes required. Some in-state travel required. May require occasional night or weekend work. May also require work on mobile medical unit for substance use disorder care in the city of Brockton.
PHYSICAL REQUIREMENTS
:
Physical demands requiring lifting of small pieces of equipment, boxes, and/or patients not to exceed forty (40) pounds. Mostly standing and walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversation with or without audio devices.

As a condition of employment, employees, to include new hires, are required to be fully vaccinated to include the most recent COVID 19 booster and annual flu vaccines, with the exception of those who have documented medical or religious exemptions, that must be approved by BNHC according to its protocols. Additional vaccines may be required based on the recommendation of the CDC. New hires who are not yet vaccinated for flu or COVID can arrange to be vaccinated on site.
