Required to draw upon Nursing Assistant's training in evaluating residents and unit needs; works to improve the care and quality of life for residents. Reports to, and takes direction from the nurse in charge on a daily basis. Position carries a Blood and Body Fluid Exposure Risk Level I. QualificationsCertification as a Nursing Assistant in Connecticut from an approved institute in long term care settings plus current Federal Registry registration in accordance with OBRA regulations. Prior experience as a Nursing Assistant in the provision of health care preferred.Qualifications:Knowledge of proper body mechanics; ability to demonstrate the use of proper body mechanics in lifting, transferring, and pivoting.Knowledge of State and Federal regulations as they pertain to nursing assistants' role and standards of care.Knowledge of cognitive disorders and techniques for working with people afflicted with them.Knowledge of infection control and safety principles in long term care.Ability to operate all equipment utilized by the class, such as Stethoscope, electric and manual beds, stretcher, geri-chair, wheelchair, lifts, etc. Physical ability to perform the functions of the class.Ability to physically complete tasks.Ability to demonstrate proper techniques for carrying out all activities of daily living in accordance with the nursing assistant registration guidelines of the Federal government.Ability to read, write, and speak English at a competency level which enables one to follow their assignment, effectively pass on information, and to comply with the first aid instructions on Material Safety Data Sheets.Job DescriptionDepartment: Nathaniel WitherellDivision: NursingBargaining Status: TeamstersSalary Range: TN-1Job Code: 4381Date Created: November 1993Last Amended: March 2023Duties and Responsibilities:Administers personal care to residents including bathing, personal hygiene, toileting, feeding, dressing, mouth care, nail care, hair care, foot care; assures that services are provided in accordance with facility policies and procedures, and with the privacy and dignity afforded to residents in the Residents' Bill of Rights.Provides rehabilitative nursing services, i.e., ambulation, range of motion, and reality orientation, in accordance with the Nursing Assistant's assignment which is based on the care plan developed for the resident; follows policies and procedures.Assures that residents assigned have been provided with their sensory and safety apparatus, i.e., eye glasses, hearing aides, safety bars, canes, etc.Receives from, and gives report to Charge Nurse, on patients at the beginning and end of each shift; reports changes accurately and promptly.Makes suggestions for nursing care plans based on observations of the resident and their response to care provided; attends care planning meeting as assigned.Supports facility policies and procedures at all times.Transfers residents from one surface to another taking care to protect the resident from injury; employs proper body mechanics in order to prevent self injury; uses mechanical lift equipment and transfer aids as needed or directed when the resident presents a difficult transfer situation, obtains assistance as needed.Maintains a clean and orderly resident unit; cares for personal belongings, keeps bedside stands and closets neatly arranged.Maintains clean bedpans, urinals, and commodes in accordance with policies and procedures.Promptly and courteously answers call bells; responds to the residents' needs and desires; refers issues to the Nurse in charge.Promptly informs the Nurse in charge of resident care problems, changes in conditions, accident or injuries to a resident, lost items, or unit problems.Changes bed linens in accordance with facility policies, and whenever needed; makes the beds of all residents assigned during the morning hours.Passes assigned nourishments, refills water pitchers in accordance with policies; offers fluids to residents throughout the shift.Passes dietary trays at mealtimes; escorts residents to the dining room; assists with applying bibs as needed; assists residents to eat their meals, or feeds residents as needed; recommends the use of adaptive aides when applicable.Notes and records intake and output of residents as determined by assignment, orients new residents and families to the unit environment, assures that call bells are within residents' reach when in bed/rooms, provides tub baths, C-tub baths and showers as directed, records charting on the COGNE with accuracy.Follows safety and infection control principles at all times; reports safety or infection control hazards to the Nurse in charge, attends at least 12 hours of mandated education programs yearly.Utilizes safety restraint procedures only in accordance with State and Federal guidelines following assignment, policy and procedures strictly; reports problems promptly.Weighs residents using standing, chair, ramp, sling, and tub scales; takes temperature readings and records findings.Transports residents as directed to designated outside locations.Provides post-mortum care.Performs other related duties as required.Participates in non-mandated in-service education programs and in studies involving the nursing unit as assigned, assists in orienting new staff to the unit and facility routines and transports residents within the facility.
