What You Will Do to Change LivesUnder direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.Shifts Available:Full Time ; 8:00am - 4:30pm ; Monday - FridayExpected starting wage range $17.30 - $22.00 Hourly. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Adults diagnosed with a serious mental illness that are resistant to mental health treatment. Family education is provided to family members or caretakers.About the Program:

Telecare In-Home Outreach Team (IHOT) is a centralized program offering mobile teams to provide in-home outreach to adults with serious mental illness (SMI) who are reluctant or resistant to receiving mental health services. IHOT offers services throughout San Diego County.The IHOT program also serves as the gatekeeper program through which referrals to the Telecare Assisted Outpatient Treatment (AOT) program are made. The AOT program is part of a continuum of supportive services offered to individuals with serious mental illness who are opposed to engaging in treatment services and who meet the nine criteria of Laura's Law.EOE AA M/F/V/Disability*May vary by location and position type
