The Population Health Services Organization (PHSO) is hiring for a full-time Clinical Resource Coordinator!Please note: This will be an onsite position.WHO WE ARE:The UVMHN (University of Vermont Health Network) PHSO (Population Health Services Organization) is a physician and payer partnered operation that provides services such as care management, care coordination, data and analytics, provider reporting, quality, risk adjustment, and value-based contract performance to enable the health network to perform successfully in High-Value Care and Value-Based Contracts.JOB SUMMARY:The PHSO Clinical Resource Partner – LPN, RT, or other licensed clinical professional works under the general direction of the AVP Population Health Services and collaborates with Population Health Services Organization (PHSO) and Medical Group teams to support High-Value Care (HVC) performance initiatives and clinical workflows. This role involves maintaining knowledge of HVC and PHSO strategy and primary care clinical workflows to support HVC performance by augmenting clinic-based care teams and clinical workflows as needed.EDUCATION:Graduate of an accredited Licensed Practical Nursing Program. Current LPN licensure or compact licensure recognized by the State of Vermont is required.ORGraduate of an AMA approved program for Respiratory Care. Licensed as a Respiratory Care Practitioner in the state of Vermont.EXPERIENCE:One to two years of experience in a relevant healthcare setting. Primary care experience is preferred. Proven customer service experience is required.
