Job Summary :

Implementation, coordination andsupervision of all aspects of a comprehensive Health, Wellness and ClinicalProgram which addresses the needs of all Assisted Living Residents inaccordance with Victory Housing, Inc. Policies and Procedures, COMAR 10.07.14and Board of Nursing Regulations and Standards.

Oversight includes

:Providenursing assessments, care planning, care oversight and quality assurance forall residents. Maintain all charts toinclude, progress notes, identification of nursinginterventions, treatments and assessments. Implementa Wellness Program to include weeklyscheduled resident Health and WellnessCheck-ups, to assurea proactive and preventative clinical approach. Oversightand coordination of all third partyvendors to include but not limited to Podiatrists, Dentists, Lab, Pharmacy,Home Health, Hospice etc. Fulfil therole of Clinical Leader in the community asan essential manager, resident advocate, case manager, delegating andpracticing RN.

: Supervision and implementation of health, wellnessand clinical programs

Areas of Responsibilities :Ensure thatthe attainment of Victory Housing’s Mission Statement takes precedence over alldecisions and actions.

Assume theresponsibilities of the Case Manager/Delegating Nurse as defined in the COMAR10.07.14 Regulations.

Responsible for all aspects ofPharmacy and Medication Management. Responsiblefor the monthly change-over of medication cards; accuracy of monthly medicationdelivery from Order to MAR; training staff on new medications, parameters,orders and special conditions; as well as updating the Emergency Sheet(electronically and hard copy). Criticalareas of responsibility are to be delegated.

In conjunction with the Assistant Director, responsibilities include interviewing, hiring, mentoring, training, evaluating and managing each MedicationTechnician. Each Medication Technician is working under the licenseof the Health/Wellness/Delegating RN.Oversight and management of medication administration must be in adherence to the Nurse Practice Act as well asFederal, State and local regulations.

Writing in all orders from physicians into the POS/MARaccording to VHI policy.

Making daily/weekly checksof MAR (EMR/Hard Copy) to ensure proper administration and documentation.

Maintain overallquality of the resident care program in accordance with all local, stateand federal regulations and the highest standards of nursing practice.

Work closelywith all staff as RN on issues. Proactively intervene whenever a status change is assessed.

Implement a comprehensiveHealth and Wellness Program designed to proactively address physical, psychosocial and rehabilitative needs of the residents. Includequality assurance and outcome measures which monitor trends andanticipate potential problem areas.

Schedule weekly Check-ups with each resident to monitor all aspects of the resident’s conditionand intervene as indicated. Review Medications, Fall Risk, Vital Signs,Weights, Nutritional Status,Skin Assessments and as well asoverall Clinical, Psychological and Social Status Changes. Record information on Wellness AssessmentForms, in Progress Notes (weekly) and Community Log (as needed).

Maintainregular communication with resident, family (in absence of ED) and physician onindividual health care issues. When astatus change is identified, be the primaryliaison with healthcare professional including pharmacy,lab and Hospice etc.

Activelyparticipate in monthly care planningmeetings, Quality Assurance Meetings, Family Clinical Reviews, and Hospicemeetings etc.

Actively participate in RSD. Report all resident statuschanges, follow-up actions and next steps. Educateas needed. Participate in

andprovide In-service Trainingas requested by ED.

Coordinate (in conjunction with ED) dietaryneeds with FSD and environmental needs with the MaintenanceTech.

Assist the ED/AD with identifying and resolving employeeconcerns, complaints,grievances and issues.

Assists the ED in completing 90-day and annual performance reviews. When requested by ED, will assist in counseling caregivers and medication technician.

Available 24/7 to support clinicalneeds of theResidents.

Direct thecommunity programs for infection control, quality assurance, outcomesmonitoring and other oversight programs.

Review all resident chartson a regular basis, checking for content, clarity, completeness andappropriateness of care. Responsiblefor the quality of all resident charts, including compliance with applicableregulations and the highest standards of nursing practice.

Instruct caregiving staffon clinical and nursing issues.

Assess each prospective Resident prior to move-in. In partnership with ED, recommend appropriateness of placementand level of care.

On day ofmove-in or within 24 hours, meet with the resident to discuss: baseline, start clinical chart and review/establish care needs and schedule incoordination with ED/AD/RSD.

Complete clinical componentsand follow-up of resident individualservice plans with the ExecutiveDirector and the caregiving staff. Completereassessments and service plans for residentsas needed. Responsible for completing all assessments as mandatedvia COMAR Regulations.

Coordinateservices of outside agencies providing health care or other assistance toresidents, including home health agencies, physical therapists, hospice,laboratories, durable medical equipment suppliers and other professionalservices.

Review andsign-off on Incident Reports with the Executive Director and AssistantDirector. Establish intervention anddocumentation of follow-up.

Attend classes,seminars and meetings.Maintain a high level of knowledge regarding current nursing theory, trendsand regulatory changes.

Scheduled hoursare from 10:00– 6:30 or 9:00 – 5:30 basedon community preference. The schedule must be set and adhered toconsistently.

In the absence of ExecutiveDirector and AssistantDirector, the Health/Wellness/ Delegating Nurse isresponsible for the day to day operations of the community.

Other duties as assigned.

JobQualifications :Must be agraduate of an accredited school of nursing with three years experience innursing, preferably in geriatric services. Musthold a current Maryland license as a registered professional nurse. Baccalaureate degree in nursing or related health field with

geriatric emphasis preferred. Must have successfully completed the 20Hour Delegating Nurse Course within 60 days of employ.Requirements:Must be current on all vaccinations including COVID- 19 andreceive CDC recommended booster doses.
