The Hospice Nurse Case Manager is an essential part of an interdisciplinary team that supports terminally ill patients and their families at home.
Using the Patient and family centered care model, the hospice nurse develops individualized care plans, evaluates changing needs, provides education and supports the family and patient with compassion.
The team's territory spans the west Baltimore County area including Halethorpe, Catonsville, Pikesville and Owings Mills.Education:Diploma, ADN and/or BSN from an accredited institutionLicensures/Certifications:*Current Maryland State license as a Registered NurseExperience:Two years of nursing experience, or equivalent combination or education and experience.Skills:*Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection*Strong time management skills*Excellent interpersonal and communication skills*Ability to be patient and family focused*Skill in the use of EMR (Electronic Medical Record) and/or ability to learn
