Assist Physical Therapists in conducting assessments and developing individualized care plans for patients.Provide hands-on, one-on-one physical therapy sessions under the supervision of licensed therapists.Collaborate with the healthcare team to ensure comprehensive patient care.Educate patients and their families on exercise routines and home care techniques.Maintain accurate patient records and document progress.Qualifications:
Valid Physical Therapist Assistant (PTA) license in the state of Illinois.Strong clinical skills and a commitment to providing exceptional patient care.Compassion, empathy, and excellent communication skills.Reliable transportation and willingness to travel to patients' homes.Experience in home health care is a plus but not required.
