Job Description
Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Heath is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a part-time (21 hours a week) Optometrist at our Patchogue, NY location.
The Optometrist provides eye care services to the patients by diagnosing and treating patient in area of specialty. Coordinates activities with other members of the health care team and with other disciplines. Emphasis is place on health prevention measures including education of patients.
Essential Duties and Responsibilities:Assumes responsibility for comprehensive eye care for patients of pediatrics, adolescents, adults and geriatrics.Renders optometry services to patients with appropriate specialist referrals and follow up.Consults with providers on SRH patients with vision complaints.Maintains responsibility for the education of all patients in proper eye care, with special concern for patients who wear glasses.Sets standards for optometric care in conjunction with the Health Center Medical Director.Renders care in compliance with SRH's care of standard.Participates in quality assurance programs and medical records audit. Aids in audit process to assure that medical standards are being met.Minimum Education Requirement:
Doctorate Degree (includes PhD's): OptometryGraduate of accredited Optometry School. Licensed to practice in area of specialty in the State of New York.
Minimum Related Work Experience:
2-4 years
Salary: $90,000 - $95,000 annual exempt for part time 21 hours a week
