Under the general supervision of PATHS’ Behavioral Health Program Director, the LCSW/LPC will provide integrated behavioral health services as part of patients’ care team alongside medical providers. These outpatient services will include individual and group sessions to address a wide range of behavioral health issues, including substance abuse.Education and/or Experience:Master’s degree in Human Services, Psychology, Counseling, or Social Work required.Active licensure as a LCSW/LPC in Virginia.Knowledge of psychotropic medications and their role in the treatment of mental illness preferred.Experience working with individuals with chronic medical conditions and concurrent mental health issues preferred.Essential Functions:Provides individual counseling and crisis intervention to adults and children who are experiencing psychological or adjustment difficulties.Provides group-counseling programs.Meets with staff of partner agencies to exchange information, provide education, and offer consultation regarding the treatment of clinic patients with mental health issues.Provides clinical evaluations to determine mental status and substance use disorders, and treatability of outpatient individuals.Facilitates linkage with crisis/emergency services when needed.Provides general behavioral health case management to outpatient individuals.Responds to partner agencies' requests for clinical input, assessment, and education on behavioral health problems and issues affecting their individuals.Provides general consultation to community agencies, including training and education for area groups, parents, teachers, and other community interest groups seeking improved skills in working with children, adolescents, adults, and families.Maintains individual records, routine correspondence, and daily statistical forms within the set time frames.Maintains confidentiality and security of Protected Health Information (PHI) as outlined in the Confidentiality Agreement.Attends and participates in administrative staff meetings.Performs other clinical and/or administrative assignments as directed by the supervisor.Provides clinical outpatient services to children and adolescents when needed.Ability to provide outpatient services within a maximum of 30 minutes per individual treated.Other Duties:Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties, or responsibilities that are required of the employee for this job.
