POSITION DESCRIPTION:

To provide direct patient care and support activities which build and maintain the outstanding outcomes of the therapy department.ESSENTIAL JOB FUNCTIONS:

Aids in the restoration and enhancement of independent function in individuals compromised by illness and/or injury. Evaluates and treats patients, communicates with families, physicians, and interdisciplinary team members, documents services in medical records. Contributes to professional development and safe working practices.Provides patient treatments:Performing accurate, comprehensive patient evaluations.Developing effective treatment plans.Supervising assistants/aides.Communicating patient progress and treatment plans.Instructing families and nursing staff.Interviewing patients and families.Demonstrating knowledge of resources and best practices.Writes accurate documentation:Records evaluations, treatment notes, progress, and discharge summaries. Manages Medicare process, acquires treatment consents/approvals.Interdisciplinary approach:Integrates team findings, patient and family input. Prepares for and contributes to conferences, collaborates with facility staff, educates team members.Professional development:Utilizes continuing education, interacts with peers, presents at conferences, maintains clinical knowledge, meets education requirements.Positive facility culture:Demonstrates core values, professional standards, contributes to a positive work team, provides superior customer service.Safe working practices:Identifies safety hazards, maintains work areas and equipment, uses proper body mechanics, safety precautions, and protective equipment.QUALIFICATIONS:Skills, Knowledge and Abilities:Knowledge in Physical Therapy evaluation and treatments.Ability to supervise and direct assistants and aides.Understanding of physical modalities, prosthetic and orthotic devices, medical terminology.Ability to work with all types of patients, excellent communication skills.Education:Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an accredited program.Experience:Experience in geriatric rehabilitation preferred. Experience in adult neurological, cardiac, and/or general rehabilitation preferred.Physical Requirements:For Direct Patient Care:Must lift and transfer patients, lift approximately 50-70 pounds, perform CPR, use hands for grasping and manipulation. Spend time bending, stooping, kneeling, reaching, standing, and walking.For Support Activities:Will spend time sitting at desk or terminal entering data.License/Certification/Registration:Current Physical Therapist Registration/State Licensure as required. Current CPR certification.
