Job: Service CoordinatorStatus: Full-TimeSalary Range: $21.00-$24.00 per hourLocation: Pasadena/Lake View Terrance SilvercrestUnit Size: 74(Pasadena) & 73 (Lake View Terrace)Perks: Paid Time Off, Full Medical and Dental Benefits, Retirement / 403b optionJOB SUMMARY:Responsible for the day-to-day identification of human service needs of residents of a Salvation Army Silvercrest housing facility, specifically related to issues of aging in place.Identifies needs and develops service plans.Identifies available services in the community and works to make them available to the building, either on-site or through individual referrals to services.Educates residents and other staff in service provision and issues of aging in place.Assists the Manager in issues related to resident independence.QualificationsExperienceBi-lingual English/Spanish preferredKnowledge of Fair Housing laws and regulations preferredKnowledge of or willingness and ability to learn general HUD program requirements for 202s and PRACs preferredKnowledge of local service agencies and resources for the elderly, knowledge of entitlement programs for the elderly preferredUnderstanding of legal liability issues relating to providing service coordination preferredDemonstrated working knowledge of working with the elderly and knowledge of the process and issues of aging, including disability, mental health and abuse issues preferred2 years: Bachelor's Degree in Gerontology, Social Work, Psychology, or related field preferred, in addition to a minimum of 2 years' experience preferred
