Job Summary:

In accordance with the organization’s mission, vision, and values, the Supportive Living Counselor/Case Manager is responsible for case management services for individuals who have been accessed as needing addictions services.Must Have Qualifications:Associates Degree in Health/Human ServicesRequired CASAC-T within 30 days of hireExcellent written and verbal communication skillsDemonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff, and community service providers.Ability to multi-task, solution-driven & time management skillsAbility to remain calm in a crisisDuties:Responsible for the development and implementation of individual client service plans, ensuring that plans are initiated; provide follow-up and modification as necessary. Assure that record-keeping is in compliance with Agency/OASAS regulations and policy & procedures as well as case management duties.Demonstrates teamwork and an ability to work in a team environment with coworkers and supervisors. Demonstrates the ability to be flexible and cooperative.Responsible for independent completion of case management duties.Facilitates community meetings and group support sessions for residents. Designs, implements, and coordinates groups based on the needs of the clients.Responsible for providing counseling to residents to assist clients in the development of appropriate interpersonal, problem-solving, and decision-making skills.Responds appropriately to crisis/emergency situations to ensure/secure the safety/well-being of the clients.Responsible for the completion of discharge activities according to policies, practices, and procedures.Assures all documentation is completed in a timely and accurate manner in compliance with agency/OASAS regulations, practices, policies & procedures.Demonstrates professionalism by providing quality service and care with respect and dignity for the clients and as a representative of FLACRA.Participate in general, staff, in-service, and supervisory meetings held by the Agency.Perform other duties and functions as directed.About Us:Finger Lakes Area Counseling and Recovery Agency (FLACRA) is a growing, not-for-profit behavioral health and substance abuse treatment provider with decades of experience and multiple innovative programs located throughout the Finger Lakes Region.
