DescriptionSHIFT: Monday- Friday 8am-4:30pmJOB SUMMARYThe Outreach SUD Counselor will work closely with the Outreach Peer Support Specialist and will provide Substance Use Disorder (SUD) mobile outreach and working closely with other homeless outreach teams and community partners to identify individuals experiencing homelessness with Opioid Use Disorder (OUD) and any co-occurring Substance Abuse Disorder/Mental Health (SUD/MH) needs.ESSENTIAL DUTIES AND RESPONSIBILITIESConduct outreach and other engagement activities to identify eligible individuals and households, including households in shelter programs and affordable housing propertiesWork collaboratively to screen and assess referrals from the Carlsbad Police Department, Carlsbad Fire Department and Carlsbad Housing & Homeless Services DepartmentWork as part of a multidisciplinary team to coordinate client careAttend and participate in regularly scheduled outreach and care conferencing meetingsCollaborate with regional Emergency Department social workers, substance use disorder counselors, and mental health practitioners to facilitate continuum of care and system navigation for clientsConduct outreach and engagement from a harm reduction approach, developing rapport while identifying risks and needsConduct in-the-field screening and provide assessment and ongoing reassessmentsDevelop personalized strengths-based plans to reduce the negative physical and psychological impacts of drugs and alcoholHelp develop informal support systems as appropriateProvide linkage to and support of engagement with recovery support services, such as vocational, educational, or crisis care servicesDevelop plans to reduce barriers to maintaining sobriety including; sober living, transportation, support groups, outpatient treatment and other needs as appropriateProvide referral and coordination into Medication Assisted Treatment (MAT), detox and other treatment optionsCollaborate across applicable agencies, providers and programs to enhance service and referral capacityProvide outreach, presentations and resources to middle and high schools that serve Carlsbad residents to provide education and programs that discourage or prevent drug misuseParticipate in youth services collaboration and refer middle and high school youth with drug related offenses or who have been identified as high risk to appropriate resourcesComplete accurate and thorough documentation in a timely mannerMaintain documentation of client meetings and interactions, including conversations and progress towards goalsMaintain regular communication with Housing & Homeless Services Department staff and provide monthly data reports of services provided and outcomes.EMPLOYMENT BENEFITSPaid Time OffPaid Holidays – 13 scheduledFlexible Work SchedulesMedical, Dental, and Vision InsuranceAD&D / Group Life InsuranceFlexible Spending Accounts – Medical and Dependent CareMatched 401(k) Retirement PlanEarly Release Days!Work Anniversary BonusesTuition Reimbursement and Public Service Loan Forgiveness EligibilityEmployee Assistance Plan (EAP)Home Ownership Assistance ProgramCasual Work EnvironmentProfessional Growth and Career Development OpportunitiesInterfaith is an equal opportunity employer.

In addition, Interfaith will provide reasonable accommodations for qualified individuals with disabilities.RequirementsEXPERIENCE / EDUCATIONHigh School Diploma or equivalent required and experience with individuals struggling with alcohol, drug, and mental illness or homelessness.Must be currently certified by the California Consortium of Addiction Programs and Professionals (CCAPP), California Association of DUI Treatment Programs (CADTP) or the California Association of Drug/Alcohol Educators (CAADE) and consistently maintain certification once obtained2+ years of experience working in an addiction treatment facility, social services or nonprofit setting required.Bilingual (English/Spanish) preferred.
