Job Objective:

A brief overview of the position.Provides clinical nutrition services to participants at the Renker Wellness Center as a component of medical programs (cardiac/pulmonary rehab and/or heart failure clinic) as well as fee for service for RWC community gym members. Responsible for coordination and implementation of nutrition care with medical, nursing, and ancillary care professionals.

QualificationsEducationRequired: Bachelor's Degree

Licensure/CertificationRequired: Registered dietician with Commission on Dietetic Registration (CDR) accreditation and American Heart Association BLS

ExperienceRequired: Two (2) years of experience as a Registered Dietician in a healthcare settingPreferred: Experience in a medical program model including Epic electronic health record; experience with cardiovascular disease, pulmonary disease, diabetes, weight management, and plant based nutrition

Essential ResponsibilitiesDemonstrates compliance with Code of Conduct and compliance policies, and takes action to resolve compliance questions or concerns and report suspected violationsAssesses the nutritional status of adult and/or geriatric patients with clear documentation in the medical record. Uses parameters such as anthropometric measurements, visual assessment and interpretation of laboratory valuesDevelops and revises a nutrition care plan based on information from the client, medical record, family and health care team membersIncorporates current evidence-based research and practice into educational materials, documentation, and policy and proceduresPerforms nutrient intake analysisAdheres to facility confidentiality, HIPPA regulations, and patient rights policiesComplies with regulatory agency standards, including federal, state and The Joint Commission as evidenced by successful completion of surveys with no deficienciesCompletes all required documentation, reports and logs in a timely, professional and thorough mannerCommunicates effectively with all team members, patients and families through conferences, individual consultation and committee participation.
