Under general supervision, plans, coordinates, and provides professional public health nursing assessment and person-centered care planning to individuals, families, and groups in home and community-based settings.

Essential Job DutiesProvides nursing assessments, person-centered care planning and referral services to individuals, families, groups, and institutions.Reviews Maryland Department of Health Long Term Services and Supports Tracking System (LTSS) assessment requests upon assignment of cases.Contacts assigned clients/representatives timely.Conducts home visits.Completes nursing assessments and high-quality person-centered care planning timely to ensure timely access to home and community-based services.Provides case management, ongoing monitoring and evaluation of person-centered care plan.Assesses and monitors clients' physical and mental states of health and determines need for community-based resources.Ensures compliance with state, federal and Medicaid regulations.Provides health screening education and counseling in the areas of disease prevention, health maintenance, and mental health.Participates in the instruction and orientation of new public health nurses.Prepares and maintains nursing records and reports.

Licenses and CertificatesPossession of a valid license to practice as a professional registered nurse, issued by the Maryland State Board of Examiners of Nurses, is required at the time of appointment. Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.
