Responsibilities:
Provide comprehensive healthcare services to patients in their homes.Conduct thorough assessments and develop appropriate treatment plans.Collaborate with other healthcare professionals to ensure continuity of care.Educate patients and their families on health management and disease prevention.Maintain accurate and up-to-date patient records.Adhere to ethical and legal standards of nursing practice.Qualifications:
Current Nurse Practitioner license in New York.Minimum of 2 years of experience as a Nurse Practitioner.Strong clinical skills and knowledge of primary care practices.Excellent communication and interpersonal skills.Ability to work independently and as part of a multidisciplinary team.Compassionate and patient-centered approach to care.
