7 AM - 7:30 PM or 11 am - 11:30 PM open to 11.5 hours 3 days/week or 7.5 hours 5 days/week

Duties:

Under the direction and supervision of the Wound and Ostomy Manager, the wound, ostomy, and continence nurse will provide safe, competent, quality care based on nursing theory and research to a designated group of patients.

Responsibilities:

-Conducts health assessments focused on wound, ostomy, and continence-related care. Performs focused physical assessment, and develops plan of care based on the diagnosis and individual client needs. Recommends treatments as appropriate.

-Assist with the development, revisions, and evaluations of standards of care.

-Assess and document patient response to wound care, ostomy, continence, fistula, and complex wound treatment. Evaluate the quality and delivery of care provided by the clinical staff.

-Works collaboratively with the health care team and makes referrals to other disciplines as indicated.

-Teach, counsels, and assists individuals and families to assume responsibility for prevention of complications as they relate to patients with ostomies, wounds, or continence problems.

-Routinely providing comprehensive assessments of patients at risk for developing pressure injuries. Initiating preventative measures.

-Participate in Unit/Department Quality Improvement and Hospital Wide Q.I activities, i.e. Incidence, Prevalence Studies and Nursing Quality.

-Provide culturally competent patient, family and staff education consistent with cultural, ethnic, religious and other belief systems.

-Maintain the security and confidentiality of data and information in accordance with patient rights policies.

-Performing other tasks as assigned.

Required Skills & Experience:

-Three to five (3-5) years' experience in healthcare.

--The above are in addition to RN contracted labor requirements.

-Ability to fully utilize BHMC's electronic record systems, equipment, and other healthcare and billing systems relevant to this position.

-Good interpersonal skills.

Education:

BSN Required

Required Certifications & Licensure:

Active NYS Registered Nurse license.

Certified Wound Ostomy Nurse (CWON) issued by the WOCNCB; or Certified Wound Care Nurse (CWCN) issued by the (WOCNCB); or Certified Wound Specialist (CWS) issued by the American Board of Wound Management (ABWM); or Wound Care Certified Certification (WCC) issued by the National Alliance of Wound Care and Ostomy (NAWCO).
