Above all, we have a genuine sense of caring and are extremely passionate about delivering our services.This Registered Nurse position is on a Part Time (various hours) working in the Perth Northern and Perth West Coast suburbs.This role is responsible for:Clinical delivery and supervision of non-clinical team members in a community settingIntake processing, clinical care planning, and reportingConducting client care assessments with a focus on Consumer Directed CareCoordinating the provision of quality person-centred care according to the client's needs, service/care plans, and available funding in accordance with budgetClinical in-home services including care plan assessment, medication, incontinence, wound, chronic disease, complex wound management, palliative care, health, and diabetes managementGeneral administration for documentation relating to client care and in alignment with AHPRA regulations and legislationOccasional on-call may be requiredQualifications

ESSENTIALQualified Registered NurseCurrent AHPRA registrationFirst Aid & CPRAustralian National Police checkProfessional Indemnity InsuranceExperience in assessing and planning care needs within the communityValid Driver's Licence and reliable, presentable carA smart mobile phone (iPhone / Android)Preferred Skills
