As professionals dedicated to Aging Life Care Management, we offer knowledge, experience, and compassion to help clients and their families with the challenges of aging and disability. Our diverse staff consists of fourteen Aging Life Care Managers (nurses, gerontologists, and social workers) who work well together and support each other. As a team, we demonstrate empathy, respect, and inclusiveness towards each other and our clients regardless of age, race, ethnicity, religion, gender, or sexual orientation.We are looking for candidates who have the following qualifications & experience:LCSW-C or LICSW, Registered Nurse, MS Gerontology, OR Physical Therapist licensureCCM certification a plusExperience in geriatrics, dementia, home health, mental health, or hospice care is requiredYou need to be a self-starter, able to work independently and communicate well with clients, families, and other professionalsThis position requires making in-person visits to our clients in the DC Metro area** At this time, we are only considering applicants who are fully vaccinated against COVID-19 to best protect the older adult population we serve**Duties & Responsibilities:Performs the initial assessment for clients.Accompanies clients to medical and other appointments.Makes regular client visits to implement care plans and monitor status. The frequency of visits (weekly, biweekly, monthly) is to be determined in consultation with the family and reviewed with the leadership team as needed.Writes client care plans and submits them to the leadership team for review.Prepares monthly written client summaries.Conducts ongoing communication with family and other service providers (includes supportive counseling for client and family). Communicates regularly with family members by phone or email, after each visit, or less often as preferred by the family.Consults with Director and other staff regarding urgent issues such as a change in health status; the need for increased services; crisis intervention; problems with communication with family members or other health care providers; and ethical issues.Develops working relationships with leaders in the community by participating in professional networking activities, and sharing information about our services.Provides occasional weekend on-call support to respond to urgent client needs or emergencies.Schedule:30 to 40 hours per week, Monday to Friday (flexible)Occasional weekend on-callCompensation:Salaried, commensurate with experienceAging Well Eldercare offers excellent benefits, including:Medical, prescription, vision, & dental coveragePaid time off & holiday leaveLong-term and short-term disability401(k) plan with matchMileage reimbursement for travelWork from your home-based office when not visiting clientsOpportunities to attend professional development meetingsPlease visit our website at www.care-manager.com to learn more about our company culture and our team!
