We are so glad you are interested in joining Sutter Health!Organization:SCAH-Sutter Care at Home - Outer BayPosition Overview:$5,000 Sign-on bonus available to external candidates - in accordance with Sutter Health policyUnder the supervision of the Clinical Manager, the Primary Care Clinician (PCC) is an experienced physical therapist responsible for the overall case management of home health patients, including the provision of direct care with a designated visit productivity standard. This position collaborates, plans, organizes, and directs all patient care services for patients in the assigned caseload in accordance with current standards and regulations, ensuring the optimal degree of quality care is maintained in coordination with an interdisciplinary team.Implements the established plan of patient care and ensures effective management of visit utilization and control of expenses. Responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. Demonstrates advanced knowledge and practice level competencies, and is a recognized clinical or workflow subject matter expert.Adheres to all local/state/federal regulations, codes, policies, and procedures to ensure privacy and safety while delivering optimal patient care.May assist in agency-wide orientation, competency evaluations, and consultation to clinical staff related to specific expertise as requested by the Clinical Manager.The Primary Care Clinician reports directly to the Clinical Manager as do the other members of the interdisciplinary team. The Primary Care Clinician may give functional guidance to Team RNs, LVNs, PTAs, OTs, COTAs, SLPs, MSWs, RDs, and HHAs.Job Description:EDUCATION

Graduate of an accredited physical therapy school.CERTIFICATION & LICENSURECalifornia Physical Therapist LicenseBLS-Basic Life Support Healthcare Provider - Issued by the AHADL-California Drivers Class CAutomobile InsurancePREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:1 year experience in primary work assignment area.SKILLS AND KNOWLEDGECollaborates with key stakeholders in developing solutions.Critical thinking, complex problem solving, decisive judgement, and ability to work independently.Knowledge and application of professional practice and regulatory requirements.Must be able to work in a demanding work environment, organize multiple priorities completing work duties within expected timelines and requirements.Computer and required technology proficiency/competencies.Demonstrates effective and efficient professional communication (verbal & written) skills as well as interpersonal skills, conflict resolution, teamwork/collaboration, customer service, and community relations abilities.Able to retain and apply new knowledge & skills.Keeps abreast of professional practice development and advancement.Ability to teach/educate others, public speaking.Ability to develop training materials and competency assessments.Develop treatment protocols.Ability to extract evidence-based practice information and apply to clinical practice and training materials.Able to analyze and identify clinical practice and workflow best practices.
