Nurse Navigator - Endocrinology

Position Summary

Under general administrative direction, coordinates referrals from health care facilities and individual providers; provides a significant level of patient education related to illness and planned management and treatment. The Navigator will act as the community liaison for support programming to ensure that the patients are aware of different services offered. The Nurse Navigator will be the point of contact for Division of Endocrinology MFA and GWUH. In addition, the Nurse Navigator provides in clinic support to providers.

As members of the GW School of Medicine and Health Sciences faculty, MFA providers are teachers and mentors for medical students, residents, fellows, and researchers preserving the rich tradition of academics, research, and healing. In addition to maintaining a closely integrated alliance with The George Washington University and The George Washington University Hospital (GWUH) which is separately owned and operated by Universal Health Services (UHS), the GW MFA has active referring relationships with 12 area hospitals.

The GW MFA’s leading healthcare presence in the DC metro region is complemented by a network of community-based practices in DC, Maryland, and Virginia. Given its geographic location in central NW Washington, DC, and proximity to more than 175 resident embassies, the MFA continues to evolve its international clinical outreach.

Essential Duties and Responsibilities

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned.

Liaison for endocrinology patients requiring coordination of care and support.

Manage all clinical issues; urgent tasks, prescription pre-authorizations and refills, appointments

Coordinate and manage all dynamic testing and procedure referrals to include patient preparation and education.

Follow up with patient on testing and procedure results in coordination with providers.

Provide appropriate clinical support, counseling, and education. Alerts provider to urgent situations that cannot wait until scheduled follow up.

Prepares Clinic prep notes for new patient and returning patient which includes scanning notes/imaging, contacting referring physicians or imaging group, and contacting patients on any issues on items needed for visit.

Prepares records needed and reconciled for future appointments.

Assist providers in addressing insufficiencies in care provision by coordinating follow up, orders, and referrals.

Participates in quality improvement, patient safety and patient experience initiatives

Measures and tracks departmental progress in meeting quality, safety and patient experience goals.

Diabetes

Keep up-to-date records/data for program and patients within MFA and GWUH.

Identify and coordinate health maintenance issues

Assist with ensuring all CGM and insulin pump data is available before in-persona and telemedicine visits.

Assist in appropriate referral to CDE or nutritionist

Assist in appropriate referral for pregnant diabetics (GDM) between OB service and Endocrine (CDE or physician)

Clinical Support

Assists providers with dynamic testing and procedure (thyroid biopsy) visits to include room setup, bedside assistance, patient education and follow up needs.

Ensures medication, allergy and problem list reconciliation is completed for all patients.

Ensures quality metrics are met for each patient.

Works with support staff to ensure rooming and provider readiness of patient

Coordinates patient discharge needs including follow up clinic appointments

Develops training and education for the Medical Assistants related to endocrinology patient populations.

Minimum Qualifications

Education

Bachelor in Nursing. Current D.C. Licensure as a registered nurse or evidence of application for licensure in D.C.

Experience

Two years of experience as a registered nurse is required.

Experience in public health or health/patient education preferred.

Experience in Endocrinology a plus.
