Access East, Inc.is a care management and practice support company providing comprehensive complex, high/rising risk, and low risk management to over 120,000 covered lives. We are a 501 (c)(3) entity that supports Medicaid, Medicare, Self-insured and Uninsured populations. Services include care management, practice support, annual wellness visits, health coaching, free clinic wrap around services, screenings/care gap closures, and SDOH coordination, just to name a few.
Position Summary
The Care Manager is responsible for coordination of the plan of care through collaboration with the provider, the interdisciplinary team, the patient and family.
The Care Manager facilitates the plan of care with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. In addition, the Care Manager helps drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements and patient satisfaction).
The Care Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of service, promote efficient utilization of resources and ensure a well-organized and safe plan of care is established for every patient.
Minimum RequirementsEducation: Associate Degree in Nursing, Bachelors of Science in Nursing, or Bachelors of Science in Social Work requiredExperience: 0-3 years of Medical Case ManagementOther Information
