In addition, you will…Identify the need for and complete a lactation assessment of patients with lactation problems or concerns.Develop and implement an evidence-based plan of care and demonstrate the ability to communicate data pertinent to the patient's health/situation.Demonstrate the necessary skills and knowledge to evaluate patient's progress, assist the family through the continuum of care and provide appropriate discharge planning.Keep accurate and complete documentation in medical records on findings and care provided.Work collaboratively with members of the patient care team to ensure optimal patient care.Demonstrate ability to perform all required competencies.Demonstrate age-specific care as established for area and job title.
