Summary:The Resident Services Coordinator (RSC) is responsible for assessing and identifying resident needs that focus on housing success. The RSC will work proactively to link elderly individuals, people with disabilities, and low-income families to supportive services and other community resources. This position must be in full compliance with HIPAA. This position will support 2 communities: Redwood Shores & St Joseph.Responsibilities:Provide non-clinical case management and referral services to all resident site(s), securing the full range of social services as needed by individual residents.Assess and identify resident needs, develop and implement individual healthy aging plans, and actively motivate and educate residents on self-management of chronic conditions, in close collaboration with hospitals and nursing homes.Conduct initial assessments within 30 days of move-in and update assessments annually or as needed.Utilize and maintain accurate and timely documentation through AASC Online. Conduct initial resident assessments and submit weekly reports to housing administrators and supervisors.Organize and coordinate on-site wellness and health improvement programs, events, and activities. Educate residents on wellness programs such as service availability, benefits, fall prevention, chronic diseases, or tools for healthy living.Conduct outreach and engagement, monitoring services periodically, and perform home visits when needed.Work closely with the Activities Coordinator (if applicable) to develop a wide range of activities designed to build community and provide opportunities for socialization and wellness.Create volunteer support programs.Collaborate and sustain constructive relationships with residents and their families, service providers, and property management staff.Maintain accurate documentation on all resident demographics, issues, requests, incidents, interactions, and outcomes through online software (AASC Online).Attend staff meetings and ongoing training or conferences.Assist residents in identifying and accessing needed services and benefits, serving as a liaison or advocate for residents.Maintain resident confidentiality according to set guidelines and report incidents of abuse and neglect as required by law.Establish partnerships with health and supportive service agencies in the community to ensure timely service delivery.Produce program materials such as a periodic resident newsletter.Other duties as assigned.Knowledge, Skills, and Abilities:Strong verbal, written, and interpersonal communication skills.Maintain professional boundaries while building trusting relationships with residents and staff.Basic computer proficiency, including maintaining data in an electronic database and using word processing software.Cultural competency in working with diverse populations.Self-direct and work independently while functioning well as part of a team.Experience building relationships with local service providers and community institutions.Understanding of affordable housing operations and fair housing practices.Ability to multi-task and establish priorities in a busy environment.Education and Qualifications:Bachelor's degree in Social Work, Sociology, Psychology, Gerontology, or related field, or equivalent experience.Experience with or knowledge of HUD regulatory requirements is a plus.Excellent written and oral communication skills required. Additional ability to speak other languages is a plus.Basic computer skills including ability to navigate the internet and use office software.Must complete annual online training and offsite in-person training as required.CA Driver License required; driving to various sites is necessary.Work Environment and Physical Requirements:The work environment is typically an office, community meeting room, or a resident's apartment. The employee may be required to stand, walk, sit, and lift/move objects up to 25 pounds.
