Position Overview:Successful candidate will have experience as a Hospital Liaison or as an RN in Home Health.
This position facilitates admissions services by providing inpatient supportive care and seamless transition to home health/hospice. Assists the organization by assuring that the right care is provided at the right place and the right time. Positively promotes Sutter as the home health/hospice provider of choice within the communities for home health/hospice services. This position is responsible for attending hospital rounds, facilitating orders, assessing patient needs, Durable Medical Equipment (DME). This position enters referral into electronic medical record and completes referral process and admits patient. Serves as a resource by supporting the mission and vision of the organization.

CERTIFICATION & LICENSURE:RN-Registered Nurse of CaliforniaBLS-Basic Life Support Healthcare ProviderAUTO-Automobile InsuranceDL-Valid Drivers License
TYPICAL EXPERIENCE:5 years recent relevant experience
SKILLS AND KNOWLEDGE:Possess critical thinking skills, in-depth knowledge of clinical disease entities common to hospice/home health and palliative care for terminally ill patient. Should also possess knowledge of hospice/home health reimbursement methods.Must have the ability to function independently and as a member of an interdisciplinary team; ability to exercise independent, sound nursing judgment in planning and providing patient care.Ability to recognize the special needs of hospice/home health patients and others in the home.Ability to communicate with patients and significant others in an effective, mature, caring manner.Ability to maintain harmonious constructive relationships with internal and external customers.Demonstrated effective written communication skills.Ability to be aware of surroundings and respond to emergency and/or urgent events. Ability to select and utilize patient transfer/transport equipment safely.Productivity standards – ability to meet productivity expectations.Must be detail-oriented.Ability to work in mentally and physically demanding environment.Ability to work in mentally and physically demanding environment.Job Shift:Days

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
