Speech Language Pathologist, SLP/ST

to join our in-house rehab team for our Skilled Nursing Facility!Facility Type: Skilled Nursing FacilityLocation: 1267 Meridian Ave, San Jose, CA 95125, USAPay Range: $55 to $62 per hourOther Benefits: Paid Training, Paid OrientationDepartment: RehabilitationRESPONSIBILITIES:Conduct comprehensive assessments to evaluate patients' speech and language abilities.Develop and implement individualized treatment plans, targeting specific speech, language, and swallowing goals.Provide therapeutic interventions, including but not limited to, articulation therapy, language development therapy, voice therapy, and cognitive training.Monitor and document patients' progress and make adjustments to treatment plans, as necessary.Collaborate with other healthcare professionals, such as occupational therapists, physical therapists, and nurses, to coordinate patient care.Educate patients, families, and caregivers about speech and language disorders, treatment strategies, and home exercises.Participate in team meetings and case conferences to discuss patients' progress and make recommendations for discharge planning.REQUIREMENTS:Valid state licensure as a Speech Language PathologistActive CPR CertificationCertificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)Is in good standing with all regulatory agencies and licensing boards is requiredInterest in working with the geriatric populationMaster's degree in Speech-Language Pathology from an accredited program.Strong knowledge of speech and language disorders, evaluation techniques, and evidence-based therapeutic interventions.Excellent communication and interpersonal skills.Ability to work effectively in a multidisciplinary teamProficiency in computer skills and electronic medical records systems.Strong organizational and time management skills.Compassionate and patient-centered approach to care.Ability to adapt to changing priorities and thrive in a fast-paced environment.EXPERIENCE:New passer/graduates are welcomeWith Skilled Nursing or Long-Term Care experience is preferredExperienced Speech Language Pathologist (SLP) or Speech Therapist (ST)BENEFITS:Our commitment to our employees includes benefit programs that are designed to meet the various needs of our employee population.
