The Case Manager, in partnership with the Santa Monica C3 Program Manager, performs a variety of tasks to assist vulnerable and chronically homeless individuals in high-density areas of Santa Monica. Responsibilities include identifying, engaging, and providing support to homeless clients, including referrals, linkages, and coordinating care, while maintaining high-quality services and operational efficiency.Essential Duties and Responsibilities

Work as part of an interdisciplinary team to engage and provide services to homeless individuals.Interface with partners, including community health providers and social service/housing providers, to reduce homelessness.Collaborate with team members to remove barriers to permanent housing.Provide case management and aftercare services to clients placed in permanent housing.Report statistics and updates for reporting purposes.Provide initial contact, needs assessment, information, education, and referrals to the homeless.Refer clientele to appropriate services (e.g., health, mental health, interim housing).Develop effective, trusting relationships with clients.Maintain accurate electronic records and client files.Attend case file, staff meetings, trainings, and retreats.Participate in field outreach in multiple locations.QUALIFICATIONS

BA degree or equivalent experience.Spanish speaking preferred.Knowledge of issues faced by the population served (homelessness, mental illness, substance abuse).Working knowledge of social services agencies in West LA.Excellent human relations/communication skills.Skilled in non-violent crisis intervention.Ability to self-motivate and organize time effectively.Computer literate; able to use computerized database systems.Passion for working with the population served.Ability to work as part of an interdisciplinary team.BEHAVIORS EXPECTED OF ALL AGENCY EMPLOYEES
