Nationwide Telehealth Dementia Care Nurse Practitioners. We are searching for full-time NP’s who are bilingual, with verbal and written fluency in Spanish. The Telehealth Nurse Practitioner in Dementia Care is an essential member of our remote healthcare team, dedicated to providing specialized care for patients with dementia and their caregivers. This role focuses on delivering comprehensive, interdisciplinary care through telehealth platforms to ensure accessible and effective treatment.Compensation & Benefits:

This position is fully remote, and we provide the necessary technology to work from home. Occasional travel to our headquarters in Chicago, IL and/or other locations may be expected.Key Responsibilities:Provide specialized, remote care and support to dementia patients and their caregivers.Assess and address the psychosocial needs of patients, families, conservators, and caregivers in a culturally diverse environment.Utilize telehealth technology to conduct assessments, monitor progress, and adjust treatment plans.Develop and implement individualized care plans in collaboration with the clinical team.Requirements:Masters of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program GraduateActive, unrestricted APRN licensure in the state you are working or able to obtain2 years of experience as an NP in primary care, internal medicine, neurology or gerontology and/or dementia care settingWorking knowledge of Geriatrics and care of Geriatric/Neurology patientsManaging a busy clinical schedule while minimizing patient wait timesComfort with multiple communication and documentation platformsClinical research experience in AD/ADRD and knowledge of current research landscape related to dementia care and treatmentBilingual, with verbal and written fluency in Spanish to support a diverse population of patients and caregivers
