Since 1976, St. Joseph Center has been meeting the needs of low-income and homeless individuals and families in Venice, Santa Monica, Mar Vista, and surrounding communities. The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through comprehensive case management and integrated social service programs. The Center enjoys broad-based community support as well as a sponsored relationship with its founders, the Sisters of St. Joseph of Carondelet. St. Joseph Center serves approximately 6,000 individuals annually.Brief Description of Principal ActivitiesMultifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles. St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.Job Description

Job SummaryThe Case Manager serves as a team member with several social service professionals and paraprofessional staff ensuring the delivery of quality services while adhering to the mission of St. Joseph Center. The Case Manager will provide case management to homeless families, including service linkage and housing support.ESSENTIAL DUTIES & RESPONSIBILITIESIn conjunction with the Program Manager and CES for Families case management staff, evaluate families for permanent housing.Be responsible for a caseload of up to 30 families. Establish a case management plan for each family.Provide referrals to any needed services including health, mental health, education, employment, and vocational training. Serve as an advocate or liaison to other organizations when needed.In conjunction with the housing specialist, assist families with the identification of appropriate housing and with lease-up including negotiation of rental agreements with landlords. On an ongoing basis, act as an advocate with landlords as needed.Provide ongoing case management to families following placement.Other duties and responsibilities may be assigned. QUALIFICATIONSKnowledge, Skills & Abilities:Must be highly motivated and a self-starter. The ability to communicate with and relate to a diverse group of people including clients, community, and other staff. Must have excellent administrative and organizational skills.Knowledge of case management principles and intervention techniques specific to homeless populations or other underserved groups.Knowledge of housing market/housing programs is desirable.The position requires an ability to work flexible hours including some early mornings and evenings.Bi-lingual Spanish/English desirable.Experience:One to two years’ experience providing case management services/advocacy with homeless and low-income families.Education:Bachelor’s degree from an accredited college or university in Psychology, Social Work, Human Services, or a related field.Direct Reports:This position has no direct supervisory responsibilities.Computer Skills:Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook).Certificates, Licenses, and Registrations:Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.Valid automobile liability insurance.Travel Requirements:To and from the work site and to other appointments as needed.The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
