OverviewWe are seeking a compassionate and experienced Registered Nurse to join our Home Health team. The ideal candidate will help us to provide high-quality care to patients in the comfort of their homes, focusing on promoting wellness and independence.
Duties
Conduct patient assessments and develop individualized care plansEducate patients and their families on disease management and self-care techniquesDocument patient progress, treatments, and outcomes accuratelyEventually may be required to have training sessions with caregivers.
Required Skills:
Knowledge of internal medicine and trauma medicine practicesAbility to provide care for patients with behavioral health needsProficiency in conducting physical examinations

Pay:
$150 per Client Visit Assessment and Plan of Care
