Full-timeGreat opportunity for an experienced family nurse practitioner to join a team of professionals proudly providing access to high quality, affordable health care to children and adults of all ages.The Nurse Practitioner is a key member of the Patient Centered Healthcare Team who participates in patient care by making assessments and helping patients manage and understand their chronic illness, acute episodes, and exacerbation.Responsibilities include:Making assessments of each patient, including presenting problem, medical history, allergies, psycho-social problems, medications, and current living arrangements.Reviewing abnormal lab values and planning appropriate disposition/treatment in absence of the primary physician.Identifying the need for client and family teaching and/or referrals.Prioritizing patient needs based on assessment and patient’s verbalized areas of concern.Developing a plan for teaching based on mutual goals that addresses patients' assessed and stated needs.Participating in a multi-disciplinary approach directed toward client’s needs and care.Consulting with other health care providers (i.e., case manager, family planning, etc.) and involving family members and/or significant others in the plan of care, as appropriate.Minimum Requirements:
