Project Permanence

program provides supports for youth to stay in their community with their families and prevent out of home placement. Our staff deliver intensive culturally responsive, trauma-informed home-based supports through four phases of service:Family Engagement : Identifying the family’s strengths and cultural experiences to determine together specific community support needs;Goal Setting : Working together to develop and implement an individualized plan of care designed to meet the goals set by the youth and family;Plan Implementation : Supporting the family in driving their plan and effectively meeting their goals;Transition : Celebrating the family’s success and strengthening links to identified community supports for sustained stability and permanence.Youth who have been in foster care, the juvenile justice system – or both – typically face great challenges as they transition back to their families. Many have experienced trauma and have been completely removed from family and their community, which has a significant impact on their ability to trust and have healthy relationships. Repairing and building relationships within the family and community is critical to well-being and a successful transition to a stable and permanent family environment.POSITION SUMMARY:

The Family Partner will team with Project Permanence Clinicians and Community Liaisons to support youth and their families in meeting their treatment goals. The Family Partner will provide insight, expertise and relevant resources to treatment team and families to support delivery of the High Fidelity Wraparound model of treatment for children, adolescents and families with mental and behavioral health challenges. The Family Partner will assist clients and caregivers in accessing resources in the community that will help the family stabilize the home environment and manage barriers to treatment.ESSENTIAL FUNCTIONS:Participate in Project Permanence team meetings and meetings with families in the home or community, such as at schools and in the juvenile justice system, as indicated by the family needs.Support caregivers and youth in identifying, creating and working with resources to sustain tasks and goals set by the Family Team.Support removing barriers to treatment by addressing needs in primary areas (home, school, work, prosocial support, economics, health/medical, legal).Document services in a timely manner according to Lincoln and county standards for billing Medi-Cal services.Participate in weekly supervision and bi-weekly group supervision.POSITION REQUIREMENTS:

The Family Partner must be able to work proactively, independently, and professionally with families to meet their case management needs. In addition they must demonstrate competence in dealing with ethnic, cultural, and socioeconomic diversity of youth and families spanning over a large geographic area. An ideal Family Partner possesses the ability to problem solve in the moment with clients and their families in order to support their development of problem-solving skills.Family Partner must possess the ability to advocate for services, empower family to self-identify, and advocate for necessary services vital to the stability of the youth at home.Experience having working knowledge of available community resources and willingness to develop community partnerships on behalf of program and clients.Experience working in mental health or social service systems.Valid California driver's license, satisfactory driving record, availability of personal vehicle and personal vehicle insurance coverage.Must meet eligibility requirements to bill for Medi-Cal services.EXPERIENCE AND EDUCATION:Bachelor's degree, preferably in psychology or social sciences, or equivalent experience.Experience working with youth and/or families, preferably parents.Professional and/or lived experience navigating multiple social systems.Experience with or working knowledge of case management.WHY JOIN LINCOLN FAMILIES:Salary: $56,160 / $27.00 per hour

Bilingual Skills:

Up to an additional $2.50 per hour

(contingent upon results of language proficiency test)
