Identify the patient’s functional weaknessesConfirm rehabilitation objectives based on the patient’s goalsDevelop a complete, patient-specific treatment planProvide continuous evaluation, adjusting the program when neededPerform initial evaluations and develop a plan of care that therapist and assistant will follow through the course of rehabilitationResponsible for writing progress and discharge notesQualifications
