Full-timeCape Cod Health Care Facility seeks a full-time Outpatient Psychiatric Nurse Practitioner to serve patients who are homeless or at-risk of homelessness, many with multiple conditions including mental health and substance abuse issues, in a compassionate, collaborative, educational, and caring environment.In this role, the NP will:Provide psychiatric mental health assessment, including description of presenting problem, psychiatric history, family history of mental health problems, relevant medical history, substance abuse history, risk assessment, etc.Provide individualized and culturally sensitive medication management applying evidence-based or best-practice standards where applicable.Provide clear documentation of all services and interventions.4-day work week option, Full benefit Package.Contact Jerome at Olesky Associates for more information.
