Registered Nurse, Case Manager - Cardiac Rehab

Founded in 1921, Straub Medical Center includes a 159-bed hospital in Honolulu, a network of neighborhood clinics and a visiting specialist program that reaches throughout the state of Hawai'i. With over 200 physicians who are leaders in their fields, Straub provides its patients with diagnoses and treatments for more than 32 different medical specialties, including bone and joint, heart, cancer, endocrinology/diabetes, family medicine, gastroenterology, geriatric medicine, internal medicine, vascular and urology. Straub is home to the Pacific Region’s only multidisciplinary burn treatment center. The hospital consistently brings new technologies and innovative medical practices to Hawai'i, such as minimally invasive cardiac surgery and total joint replacement.Under the general direction of the Director of Cardiac Services or designee, the Registered Nurse, Case Manager - Cardiac Rehab is responsible for actively coordinating and monitoring all clinical aspects of the Program intervention. This includes ensuring the appropriate selection of participants and monitoring participant safety with active oversight of the Medical Director. The role also involves assessing participant’s clinical response to the interdisciplinary treatment plan and funneling communication to the appropriate specialist and/or Medical Director or referring physician. The Registered Nurse will provide leadership in weekly case conferences and assure safe care transition of participants upon graduation. Additionally, the nurse will function as a superuser of the electronic health record, DiaWeb, and participate as a member of the transdisciplinary team to ensure the quality and clinical effectiveness of the Program.Work Schedule:

Current Hawai’i Registered Nurse license. Bachelor's Degree and/or equivalent combination of education, training, and/or experience. Current American Heart Association Advanced Cardiac Life Support (ACLS) CPR card. Two (2) years clinical experience in adult cardiovascular nursing and/or cardiac rehabilitation.Preferred Qualifications:
