Essential Duties and Responsibilities:Develop and implement best practices in patient safety and risk mitigation initiatives driving a proactive approach and prevention of patient harm.Supervises the investigation of errors and plans for resolution of all safety-related incidents, including those involving employees, patients, and situations posing a threat to the healthcare setting and equipment.Maintains records of patient-related errors and evaluates this information to identify the cause of the error.Collects, reviews, and analyzes information from the facility's safety programs to identify corrective actions and programs as necessary.Recommends changes, policies, or programs that could prevent future errors.Designs and presents training programs meant to increase awareness of patient safety initiatives.Ensures compliance with state and federal regulatory requirements.Typically reports to a head of a unit/department. Manages subordinate staff in the day-to-day performance of their jobs.Ensures that project/department milestones/goals are met and adhere to approved budgets.Facilitates the implementation of safety through policies, procedures, and guidance, serving as the facilitator for program sites providing a strategic, investigative approach in monitoring daily tasks and activities.Reviews and analyzes facility occurrence reports to identify clinical risk issues.Conducts root cause analysis, apparent cause analysis, incident analysis, and healthcare failure mode and effect analysis.Assists with regulatory survey and planning as needed.Facilitates the investigation and follow-up on events with the appropriate Directors.Analyzes occurrence data for frequency and severity of events.Identifies trends necessitating intervention and works with Executive staff in planning and implementing actions to address identified issues.Initiates Probable Claims Report (PCR) as appropriate.Coaching, counseling, conflict management, consulting, and influencing skills.Other duties as assigned.Qualifications:Clinical experience required; Bachelor’s Degree required, Master’s Degree preferred.Certifications: CPPS, CPHQ, or CPHRM designation preferred.Demonstrable experience with relations required.Ability to work independently and maintain accountability for work.Must have excellent computer skills, including word processing and spreadsheets.Ability to provide leadership and direction within duties.Must be able to maintain acceptable attendance and adhere to scheduled work hours.Experience:

This position requires a minimum of two to five years of experience in acute hospitals and/or integrated healthcare delivery systems and a broad knowledge of hospital operations. Extensive experience in patient safety and risk management is required.
