You will plan and coordinate the care of the patient from pre-hospitalization through discharge, and work with all members of the health care team to assure a collaborative approach is maintained in care and treatment of the patient.Requirements:Must Have Epic ExperienceMust Have recent acute care experienceExperience with Medical/Surgical/Telemetry/ICU/Peds/NICU/PICU/OBBLS requiredMinimum of two (2) years of case management experience or three (3) years of acute hospital experience, will accept 1st time Traveler if coming from Level I, large facilityEducation:Associate's Degree in Nursing, MinimumBachelor's or Master's Degree in Nursing, PreferredLicensure:Current CA Registered Nurse Licensure, RequiredCertified Case Manager CCM or ACM, PreferredFor prompt and confidential consideration, please apply to the link below:Click here to apply online
