We advocate for and create affordable housing to end homelessness in our communities."PurposeUnder the direction of the Program Manager of Case Management Services, the Outreach Case Manager seeks to outreach and engage men and women over the age of eighteen who are homeless and struggle with unresolved issues of mental illness and substance abuse and are not receiving services and to link them to agencies and activities that will stabilize their lives, support their sobriety, and empower them to end their homelessness.Duties and ResponsibilitiesConduct street outreachProvide linkages for clients who are in need of social, mental health, educational, vocational, recreational, spiritual, and/or substance abuse servicesProvide transportation to appointments for clientsCoordinate and collaborate with other agencies and social service providersEngage in “street” outreach in all kinds of weather conditionsAttend weekly and monthly agency, program and network meetingsMonitor all cases assigned and complete all documentation/charting required by Columbus House and its relevant fundersComplete First Contact FormsMaintain and update weekly charting and movement reportsEnter and maintain all client charts accurately and timely into the Homeless Management Information System database Case WorthyOther related duties as assigned in support of a 17 client case loadQualificationsMinimum of two years’ experience working in human services or related fieldsExcellent organizational and time management skills with a deadline oriented mindsetStrong written and oral communication skills employing the English languageProficient in Microsoft Office and other common desktop applicationsAble to effectively use the local network of substance abuse, mental health, and social services providersFamiliar with criminal justice and able to engage, manage, motivate, and supportFamiliar with mental health, substance abuse and dual diagnosis issues and group dynamicsComplete knowledge base of all aspects of the delivery of person-centered case managementAble to meet all documentation requirementsKnowledgeable about crisis intervention, boundaries, confidentiality, universal precautions, harm reduction, trauma informed care and motivational interviewingValid drivers’ license with a good driving historySupervised by:

$25.50-$27.50 per hour based upon education and experience plus excellent benefitsSchedule:
