After 1 year of active employment Maintain at least 0.20 FTE (8 hours per week)POSITION SUMMARY: This position provides basic care for residents to assure the highest degree of quality resident care is maintained at all times within a safe environment; assists with activities of daily living, offers emotional and social support, and attends residents overall physical comfort.ESSENTIAL FUNCTIONS: The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. To perform this job successfully, an individual must be able to perform each duty satisfactorily. Other duties may be required and assigned. Provides personal hygiene including bathing (tub, shower, bed bath), oral care, skin and nail care and hand washing. Completes resident grooming including hair care, shaving, dressing and weighing. Encourages residents toward independent personal care. Assists with toileting including bowel and bladder retraining and perinea care. Performs and assists with mobility, transferring, ambulation, positioning and range of motion per the care plan. Assists in orienting residents upon admission and throughout stay as needed. Documents accurately all care given, including dietary intake and output, temperature, weight, blood pressure pulse and respiration, using approved abbreviations and descriptive terms. Reports to nurse on units for written and verbal assignments which include daily use of care plans for assigned residents before initiating care. Participates in the planning of resident care through shift reports and resident care staffing meetings when assigned. Informs verbally and/or submit in writing any observations and changes in a resident for the licensed nurse to evaluate including dietary intake. Serves residents their meals; motivates and assists them with intake of nourishment and fluids. Answer all call signals and requests of residents immediately. Encourages/assist residents in keeping their rooms clean and orderly; assists with making beds. Maintains nursing unit in a clean and orderly manner. Keeps all residents' personal belongings and equipment clean. Escorts residents to appointments and/or therapy departments as scheduled. Participates in unit activity programs planned for residents as needed. Attends and participates in unit nursing staff meetings and mandatory in-services. Performs a total inventory of all resident's possessions on admission, discharge or transfer. Ensures all residents' possessions are properly labeled. Responds to resident emergency including bleeding, falls, seizures and performance of Heimlich maneuver. Assists with post-mortem care as assigned. Transports dietary carts and linen carts. Other duties as assigned. KNOWLEDGE, SKILLS AND ABILITIES Knowledge of PPE and its proper use for personal safety measures. Ability to maintain confidentiality of all resident care information and assure resident rights and privacy are protected at all times per HIPAA. Ability to create and maintain an atmosphere of warmth and personal interest as well as a clean environment. Ability to work tactfully and cooperatively with residents, families, visitors and entire staff throughout the organization. Ability to verbally communicate with residents, families, the public and other members of the organization. Must be able to exhibit a warm, cheerful, caring manner. Must be able to safely perform the essential job functions. General knowledge of standard office procedures, practices, equipment and office assistance techniques. Knowledge of County and department programs and policies. Ability to prepare and maintain accurate and concise records and reports. Ability to work the allocated hours of the position. LANGUAGE SKILLS Ability to communicate appropriately/professionally. Ability to communicate effectively with other members of the staff, supervisor, and the public. Ability to communicate clearly and concisely in both written and verbal form. Must be proficient in workplace English and spelling. Ability to read County policies and procedures; written instructions, general correspondence; SDS sheets, safety manuals, maps, etc. MATHEMATICAL SKILLS Ability to calculate mathematical calculations. REASONING ABILITY Must be able to remain calm when called upon to assist residents during emergencies. Ability to understand and effectively carry out verbal and written instructions. Ability to develop, interpret and implement local policies and procedures; written instructions, general correspondence; Federal, State, and local regulations. Ability to define problems and deal with a variety of situations. Ability to think quickly, maintain self-control, and adapt to stressful situations. Ability to maintain discretion regarding business-related files, reports and conversations, within the provision of open records law and other applicable State and Federal Statutes and Regulations. Organizational and time management skills needed to meet deadlines. Must have ability to work accurately with attention to detail. Ability to maintain confidentiality. Ability to prepare and maintain accurate and concise records and reports. Ability to analyze facts and to exercise sound judgment in arriving at conclusions. PHYSICAL REQUIREMENTS This work requires the occasional exertion of up to 75 pounds of force; work regularly requires sitting, frequently requires standing, speaking or hearing, using hands to finger, handle or feel and repetitive motions and occasionally requires walking, stooping, kneeling, crouching or crawling and reaching with hands and arms. Work has standard vision requirements. Vocal communication is required for expressing or exchanging ideas by means of the spoken word. Hearing is required to perceive information at normal spoken word levels. Work requires preparing and analyzing written or computer data and observing general surroundings and activities. Work frequently requires exposure to bloodborne pathogens and may be required to wear specialized personal protective equipment. May work in any area of the facility. May be subject to exposure to infectious wastes, diseases, and medical conditions. Vaccinations and testing as required by local, state, or federal regulations and/or other regulating entities. WORK ENVIRONMENT Work is primarily in a health care setting. MINIMUM QUALIFICATIONS: EDUCATION AND EXPERIENCE REQUIREMENTS Age 16 or older. Successful completion of CNA training course. Wisconsin certification as a Certified Nursing Assistant and on state registry. Must successfully pass criminal and caregiver background check.
