Alaris Health at St. Mary’s Subacute Rehab and Ventilator Care Unit, a 188 bed-skilled nursing Center in City of Orange, New Jersey, seeks a social services director to plan and administer social service programs in the Center and work with an interdisciplinary team to transition residents back to their homes or community or to a long term care setting. Additional responsibilities include:
supervising staff and assisting in developing community planning for health and welfare services
ensuring that residents are provided medically related social services to maintain or improve their control over every day physical, mental and psycho-social needs
documenting each resident’s conditions upon admission and coordinate the discharge planning process
performing an evaluation of and documenting each resident’s condition upon admission and subsequently creating a customized total care plan
liaising between residents, their families/responsible parties, outside agencies and the Center administrator to ensure that each resident’s rights are maintained
JOB REQUIREMENTS:
Bachelor’s degree in social work or the human services field (master’s degree preferred)
Licensed according to New Jersey state regulations (LCSW)
Excellent interpersonal skills, highly developed English language skills and knowledge of all governmental regulations related to the social services department
Ability to interact and utilize community resources to achieve positive outcomes in the discharge planning process
Comfortable working both as an individual contributor and part of a larger team
