Provide direct medical care to PACE (Program of All-Inclusive Care for the Elderly) participantsComplete sections of assessments and care planningManage a panel of participantsInitiate, evaluate, review and change orders for medications and diagnostic studies or treatments with approvalAssess, initiate and coordinate care on a daily basis under supervisionDocument all participant interactions, assessments, next steps in the EMR within the specified timeframe set by the centerMake referrals to specialists with approval and request consults as neededPerform history and physicals and participant teachingOrder, interpret and follow up on lab tests and resultsPerform selected proceduresRecognize and escalate abnormalities as neededLiaise with other team members for participant’s care, including behavioral health and pharmacyEvaluate and treat participants where they are located, whether that be in the center, home, virtually, SNF etcFacilitate advanced planning discussions with participantsDocument chronic conditions via HCC captureProvide on-call coverage as neededRequirements

Unrestricted California Nurse Practitioner License with Prescriptive AuthorityMaster’s degree and graduate of accredited NP programBilingual in English and SpanishCPR CertificationPassion and mission orientation for serving high-risk seniors and frail older adults; experience with complex and older populationsStrong communication and interpersonal skills; ability to lead independently while also working collaboratively with others in a team settingExperience as an RN for 2 years with complex populations, prior to becoming an NPCurrent knowledge of chronic care/geriatric issues and best practicesProof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law
