Job Summary

PACE Lafayette (Program of All-Inclusive Care for the Elderly) is a nonprofit health and supportive services program designed to assist seniors who want an alternative to long-term care placement. PACE is sponsored by the Franciscan Missionaries of Our Lady Health System (FMOLHS)-the parent company of Our Lady of the Lake Regional Medical Center-the Centers. The mission of PACE Lafayette is to enable the aging population to live with dignity in their communities. With PACE's assistance, participants can remain in the secure surroundings of their own homes and communities while providing family members and caregivers with much needed professional guidance, physical relief and emotional support.

Our team works together to develop individual care plans, goals and carefully monitors progress. Our multi-skilled team-consisting of a doctor, registered nurse, social worker, physical therapist, occupational therapist and registered dietitian-works together to develop patient goals, plan care and carefully monitor progress.

Job Summary

The driver operates 14 passenger vans or smaller. The driver transports Participants to and from their own home to the PACE Center and to Physician appointments. Prepares and provides appropriate documentation as required. May be responsible for vehicle maintenance and presentation. The position relies on established guidelines to accomplish tasks and works under close supervision

Responsibilities

Driving Duties

Drives a specially equipped van to deliver physically limited patients/residents to varied destinations. Assures that wheelchair lifts and tie down equipment are properly operated and that patients/residents are delivered to appointments in a timely manner. Assures that all patients/residents are in seats and have seatbelt properly fitted and secured.Follows established schedules, adjusts schedules to meet unanticipated changes, determines the safest and most efficient pick-up and delivery routes.Assists patients/residents on and off the van in a safe and courteous manner. Deals with medical equipment and other emergencies that may arise.Takes on added responsibilities of picking up secured medication from the pharmacy as scheduled and will assist in delivery of sealed packaged medications to the nursing homes.

Vehicle Monitoring/Inspections

Inspects van daily utilizing van checklist sheets. Reports the need for maintenance.Maintains van in a clean, orderly and safe condition.Maintains accurate and time maintenance and repair logsProvides documentation of daily trip logs, mileage and other data required for DOTD monthly reports.Responsible for safe operation of the OSBM, Inc. vehicle and is responsible for oversight of keeping the van with adequate fuel, to avoid "running out of gas" while in transport with or without patients/residents.

Member of the Interdisciplinary Team

Attends staff meetings and takes part in facility in-services. May be required to perform Transportation Assessments.Communicates any change in patients/residents' condition to nursing department.Completes documentation as required.Demonstrates willingness to assist staff with direct patients/residents' care /activities as needed and as trained, when not driving the van.

Qualifications

Experience - 5 years of good driving experience: no more than 2 moving violations in the last 3 years and zero DUI convictions or reckless driving convictions; periodic motor vehicle/ driving record checks may be obtained throughout employment. One year experience transporting individuals with special mobility needs. One year experience with the frail and elderly population.

Education - High School Diploma or GED

Special Skills - Must be able to work effectively in a team environment and operate handicap accessible vehicles.

Licensure - LA Driver's License CDL Driver's License
