Job Summary: You will carry out the duties of a Navigator for Supported Decision-Making clients. You will follow clients leaving the prison system in an effort to prevent plenary Guardianship/Conservatorship. Qualifications and Requirements: Degree in Nursing, Human Services, or other relevant field or two years of experience in a closely related field with similar clients and similar decision-making capacity. Willingness to become an Advanced Care Planner to assist in completion of Healthcare Directives. Knowledge of public benefits. Valid driver's license and DHS/BCA background check. Extreme detail attention to communications, accurate and timely documentation, and billings. Clear and unbiased thinking and ability to support clients' desires as long as they are legal and do not cause harm. Assessment skills. Comfort with ambiguity.
