We are an academic home health agency, providing care in the community for almost 30 years. We serve Central Virginia, including Charlottesville, Albemarle, Augusta, Rockingham, Harrisonburg, Staunton, Waynesboro, Nelson, Buckingham, Fluvanna, Louisa, Orange, Madison, Greene and Culpeper. UVA Health is Magnet recognized, Medicare certified, and accredited by the Joint Commission.Our team works collaboratively to develop and implement a patient-centric plan of care for each patient. We work closely with providers at UVA Health, and in the community, to ensure the safe delivery of care in the home. Our RN Administrative Coordinators are responsible for the implementation of the home health program, including quality review, quality improvement, intake and referral review. American Heart Association (AHA) Health Care Provider BLS certification required.PHYSICAL DEMANDSJob requirements: None required. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs.
