We are seeking a
Home Health Clinical Team Manager
to serve as a leader and resource for the Home Health Department on a day-to-day basis. This role is responsible for coordinating high quality patient care using an interdisciplinary team approach.

Essential Duties & Responsibilities include
(but are not limited to):
Provide guidance to the team members in regular transdisciplinary team meetings; the development, review and revision of the patient/family plan of care; facilitation of problem solving sessions; and delivery of team support.
Assess daily staffing needs and implements re-distribution of resources based on census to maximize patient care.
Periodically audit selected clinical records for documentation concerning HHA supervision, visits/matching orders, and other areas related to regulatory compliance and enter findings in audit database. Hold staff accountable to standards.
Coordinate transdisciplinary team meetings and supervise the planning, coordination, and delivery of the transdisciplinary team interventions.
Education/Experience :
Bachelor’s Degree in Nursing
Minimum of two years of nursing experience ideally in a home health, hospice or community health setting within the past five years.
Five years supervisory experience or relevant progressive management experience preferred
Certificates, Licenses, Registrations:
Current license as a Registered Nurse in the State of California
Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
Pay Range:$130K - $156K annually
