What You’ll Be Doing:
Responsible for coordinating continuum of care and discharge planning activities for a caseload of assigned patients.Coordinating all facets of a patient's admission/discharge.Performing utilization review activities, including review of patient charts for timeliness of services as well as appropriate utilization of services.Ensuring optimum use of resources, service delivery, and compliance with external review agencies' requirements.Act as consultants to the clinical team, service lines, and other departments regarding patient assessment and patient care and participate in program development and quality improvement initiatives.Apply guidelines and collaborate with multidisciplinary teams, influence and direct the delivery and quality of patient care.What You Need to Bring to the Table:
Active and unrestricted California RN licenseBachelor’s Degree in Nursing preferredCurrent BLS certificationTwo (2) years of progressively responsible and directly-related work experiencePrevious Case Management experience, preferably in an Acute Care and Emergency Department Nursing Experience settingExcellent verbal and written communication skillsExcellent computer skillsWhat’s In It For You?
