About this role:

Our pediatric Nurses are part of a team of specialists that provide family-centered programs that support and enable families to meet their child's individual needs through education, training and support. You will work with families in a hybrid of telehealth and in-person capacity to provide services. Our nurses work with children ages 0-3 in this program.

Schedule:

Monday - Friday, 8 am - 4:30 pm

We are looking for RN's who:

Possess current licensure as a Registered Nurse by the Massachusetts Board of Registration in Nursing.Must have a valid driver's license and reliable transportation.Pediatric experience.Meet DPH requirements.The physical requirements for our Nurses:

Frequent movement within classroom including but not limited to: kneeling, bending, sitting, lifting, and positioning of childrenOperation of adaptive and specialized equipmentAbility to lift 50 pounds alone, 51+ pounds with the assistance of another person or adaptive equipmentAbility to physically assist individuals as determined by their needsAbility to tolerate significant amounts of walking, standing, and sittingAs a KDC EI Nurse, you'll get to:

Partner with families to identify each child's strengths and developmental needs. Together develop an Individualized Family Service Plan (IFSP).Work closely with a transdisciplinary team on program planning and delivery of services for families by providing intake, screening, assessments, and ongoing service coordination according to agency and DPH standards.Provide a minimum of 25 hours of direct services for children and familiesOversee case management and referral services to assigned caseload, including participation in interagency case conferences.Conduct home visits to provide developmental stimulation, parent training, and supportFacilitate parent/child and/or child groups to help children thrive within their family and communitiesComplete reports of assessments, progress reports, IFSPs, and discharge summariesMaintain appropriate communication with physicians, clinics, and community agencies participating in each child's careProvide information to parents that enable them to make informed decisions for their child's progressThis role allows for flexibility in creating your own schedule to visit with families
