Enjoy a competitive salary and mileage reimbursement, with the final hiring decision made by our client agency.Key Details:Salary: $82,500 - $97,800 Per Year, Depending on ExperienceBenefits: On-call stipends, Mileage reimbursementType: Field roleHiring Decision: Made by client agencyResponsibilities:Assess patient conditions and develop individualized care plans focusing on comfort and quality of life.Maintain detailed medical records and communicate patient status to relevant parties, ensuring seamless care coordination.Serve as the primary liaison among patients, their families, and healthcare providers, offering compassionate support and education.Educate patients and families on health management, palliative care options, and preventive measures to manage symptoms effectively.Coordinate with healthcare professionals to ensure continuous, patient-centered care and efficient case management.Conduct regular home visits to monitor patient progress and adjust care plans as necessary.Requirements:License: Active RN license in Arizona with no restrictions.Experience: At least 1 year of in-home hospice experience is required.Skills: Proficient in case management software, exceptional organizational and communication skills.Traits: Empathetic, detail-oriented, and able to operate autonomously in field settings.
