He/she is primarily responsible and accountable for maintaining an accurate nursing visit schedule that is complete and communicated to all staff on a daily basis. This includes assigning all start of care visits to both an admitting nurse as well as case manager. He/she enters this information into Horizons Home Care and provides communications to these individuals as appropriate. Additionally he/she is responsible for ensuring all ongoing patient visits are assigned to a clinician to be completed and the appropriate visit service is generated in the Horizons Home Care system. He/she further is responsible for generating all visit frequencies entered by non-case manager clinicians to facilitate clinician scheduling whenever possible.
